I could not find one that did what I wanted - a MULTI PASS card holder that takes a credit card size plastic card. Some were larger, some were close, but the card stuck out. One was slightly distorted! Watching the film it looks like it is a credit card sized card in a holder!
I have looked at a lot of photos and watched the film again, and I think this is pretty good...
This model takes a standard credit card size plastic card.

Three files: the whole thing (unlikely to print); Base with alignment pens; Top with alignment holes. Easy to assemble with a spot of glue.
It is in OpenSCAD, and has a load of variables and comments so you can tweak the design if you need.
